Activating your online account

At PuntP we use Minddistrict as our digital care platform. You can access it on a computer, tablet, or through the app on your mobile phone. If you are a client at PuntP, you will automatically receive an invitation to join Minddistrict. You can read more about this on the Minddistrict information page. In Minddistrict, you will also complete the digital pre‑intake: this questionnaire helps you prepare for your intake appointment. It is extremely helpful for both you and us if you fill it in. This way, we have enough time during the intake to focus on your story.

Completing questionnaires before the intake appointment

Two questionnaires complement the intake appointment and help us understand your symptoms as accurately as possible. These questionnaires will be sent securely to your email address within 24 hours after scheduling your intake, with PuntP as the sender. If you haven’t received the email, please check your spam folder. Once you complete the online questionnaires, the results will automatically be added to your file.
